You will require this declaration if you intend to operate a hospital where patients may claim expenses from the Pharmaceutical Benefits Scheme, Medicare Benefits Scheme or private health insurers.
You are required to apply to the Minister to make a declaration in regards to your hospital in the following circumstances:
- Commencement of a new facility;
- Change of existing hospital's registered business name;
- Change of existing hospital's address; and
- Amalgamation of existing hospitals.

A licence defines the need to obtain recognition / certification and registration to undertake a certain business activity.
To gain a declaration you must provide a:
- Private hospital information form;
- Copy of your state/territory hospital licence; and
- Copy of an accreditation certificate or evidence that the accreditation process has commenced.
