Definition of terms

Private health facility-day hospital

a facility at which day hospital health services are provided to persons who are admitted to, and discharged from, the facility on the same day (not including a facility operated by the State).

Private health facility-private hospital

a facility at which health services are provided to persons who are discharged from the facility on a day other than the day on which the persons were admitted to the facility (not including a hospital operated by the State, a nursing home, hostel, or other facility at which accommodation, and nursing or personal care is provided to persons who have a permanent need for nursing or personal care).

Eligibility requirements


Before submitting your application approval is required according to Part 5 of the Private Health Facilities Act 1999 and as contained within the Application Guidelines available on the Department of Health website.

Transferability

This licence can be transferred

You will be required to give details according to the application guidelines for transfer of a licence.

You can transfer this licence by completing an "Application for Transfer of a Private Health Facility Licence" application and submitting it to the Department's head office at the address specified on the application form.

Before the licence will be transferred an on-site audit of the facility will be conducted to review the premises and equipment in accordance with the Act and Regulations. A written report detailing any requisitions will be provided.

Duration

This licence remains in effect for up to three years.

Application

You can obtain this licence by completing a "Licence to Operate a Private Health Facility (Licence to Operate)" application and submitting it to the Department's head office at the address specified on the application form.

Forms are available from the agency or from the Department of Health website. You should contact Private Health Regulation prior to application.

Additional information as required by the Chief Health Officer following assessment of the application against the guidelines may be required if insufficient information is provided.
