You will require this licence if you intend to transport bulk controlled waste, or a packaged controlled waste on a road in Western Australia. Controlled waste refers to waste that is produced by, or as a result of:
- an industrial or commercial activity
- a medical, nursing, dental, veterinary, pharmaceutical or other related activity
- activities carried out on or at a laboratory
- an apparatus for the treatment of sewage.
Controlled waste includes
- food processing waste.
Packaged controlled waste is a controlled waste transported in a container. Bulk controlled waste includes all liquids that are transported in a bulk tanker.
Individual drivers transporting bulk controlled waste will be required to apply for an individual driver licence. The owner of tanks transporting bulk controlled waste will be required to apply for a separate controlled waste vehicle licence for each tank.
When transporting controlled waste, you will require a controlled waste tracking form. Each tracking form represents one truckload of controlled waste and is valid for one disposal only. A controlled waste tracking form is used to track the controlled waste from point of generation to point of disposal. For controlled waste carriers, a controlled waste tracking form is required when you transport 200 kilograms or litres or more of controlled waste.

A licence defines the need to obtain recognition / certification and registration to undertake a certain business activity.
To be eligible for this licence you must:
- Submit the required forms;
- Provide a copy of your certificate of incorporation or a copy of your certificate of registration of business name if applying as a company;
- Provide details of your spill management plan;
- Attach a confirmation letter from the proposed disposal site that they are capable of accepting the controlled waste of the category you are transporting; and
- Pay any required fees.
