You will require this permit if you provide health or veterinary services and intend to purchase medicines or poisons for use as part of your business.
Health services include medical centres, medical practices, day surgery units, ambulance and paramedic services, anaesthetic services and residential aged care. Veterinary services include the operation of a veterinary clinic, veterinary practice, or veterinary hospital.
This permit will authorise you to use poisons listed in schedule 4 and / or schedule 8 of the poisons Act. Schedule 4 poisons are prescription only medicines and prescription animal remedies. Schedule 8 poisons include drugs of addiction.
Your permit will specify the types of poisons which you are authorised to use.

A licence defines the need to obtain recognition / certification and registration to undertake a certain business activity.
To be eligible for this permit, your health or veterinary practices must have prior approval to operate.
You are required to develop, implement and maintain suitable systems within the facility which restrict access, ensure appropriate storage occurs and that records of use are maintained.
The poisons listed may only be used for the purpose stated on the permit by the permit holder or persons under his/her direction. They may not be supplied or resold.
