Public Health Risk Activity Licence - Community Pharmacy - Australian Capital Territory

Description

You will be required to apply for this licence if you intend to operate a community pharmacy in the Australian Capital Territory. A community pharmacy is any pharmacy that is located outside of an institution. You will not require this licence if you operate a pharmacy inside any of the following institutions:

  • A hospital;
  • A residential aged care facility;
  • A residential disability care facility; or
  • Any institution used for the accomodation, treatment and care of people suffering from mental or physical conditions.

A licence defines the need to obtain recognition / certification and registration to undertake a certain business activity.

Eligibility requirements

To be eligible for this licence you must be a pharmacist, a complying pharmacist corporation or a former corporate pharmacist. You are also required to submit a completed and signed application form providing:

  • Details about your intended community pharmacy;
  • Details about the premises that you intend to operate your community pharmacy from; and
  • A detailed sketch plan showing the layout of all fixtures, fittings, appliances and any other equipment to be installed on the premises.

Remains in force until suspended or cancelled.

The application must include the signature by the applicant, the specifics of the public health risk activity, the premises on or from which the applicant intends to carry on the public health risk activity and accompanied by a sketch plan of the premises.
