Human Artificial Fertilisation Exemption - Western Australia

Description

You may require this exemption if you are a medical practitioner and you intend to carry out an artificial insemination procedure.

This exemption authorises you to carry out a particular artificial insemination procedure without being required to hold a Human Artificial Fertilisation Practice Licence.

A licence defines the need to obtain recognition / certification and registration to undertake a certain business activity.

Eligibility Requirements

To be eligible for this licence you must:

  • be a current registered medical practitioner
  • notify the Agency of the kind of procedures you intend to carry out
  • lodge a written undertaking to observe and comply with the Code of Practice and any directions
  • pay the required fee.

You will not be eligible if you are an individual or organisation that is bankrupt, under receivership, in liquidation, or disqualified from holding a licence.
