The statement must be provided as a plan that outlines:

  • the place or object (heritage site) that may be affected
  • the proposed activity including the reason for, extent and duration of the activity
  • the likely impact of the activity on the heritage site
  • the measures you will put in place to reduce the risk of diminishing the heritage significance or damaging the heritage site
  • whether there are other methods that could be used to carry out the activity.

You will also need to provide your personal identification details with your application.
