Approval for a Private Health Facility - Queensland


If you propose to operate Private Health Facilities you must first hold approvals.

A licence defines the need to obtain recognition / certification and registration to undertake a certain business activity.

Eligibility requirements

Pre-requisites to your application are contained within the Application Guidelines, which are available on the Department of Health website.


Approvals remain in force for not more than two years. An application to extend the term of the approval by a further two years may be made.


Application process

You can obtain this licence by completing an 'Application for Approval for a Private Health Facility' application and submitting it to the Department's head office at the address specified on the application form. Forms are available from the Department of Health website.

You should also contact the Chief Health Officer prior to application.

Additional information as required by the Chief Health Officer following assessment of the application against the guidelines may be required if insufficient information is provided.


This licence cannot be transferred.
