All applicants must submit the following to the Office of Liquor and Gaming Regulation (OLGR):

You must also provide the following depending on whether you are applying as a body corporate (hotel, club) or individual licensee.
Body corporate applications
If you are applying as a body corporate (e.g. hotel or club), you must also complete one of the following options:

Form 3B: Club details for a category 2 gaming machine licence

Form 3C: Company details for a category 1 gaming machine licence (hotel)
Individual licensee applications
If you are applying as an individual licensee you must also complete the following options:

Vary
Once you have a gaming machine licence (hotel or club) you can apply for the following:
Increase in the approved number of gaming machines

Form 7: Application to increase approved number of gaming machines
Decrease in the approved number of gaming machines

Form 7A: Application to decrease the approved number of gaming machines
Increase in approved gaming hours (permanent)

Form 70: Application for an increase in approved hours of gaming
Increase in approved gaming hours (temporary)

Form 10: Application for one-off extended trading hours
Permanent or temporary transfer of club entitlements

Form 77: Application for a permanent transfer of gaming machine entitlements between clubs

Relocating premises (hotels)

Form 3G: Application for a replacement premises by a category one (hotel) gaming machine licence
Relocating the approved gaming machine area

Change of club committee or executive officers of a company

Disposal of electronic gaming machines
OLGR no longer requires notification of the disposal of gaming machines. However, following a decrease in the approved number of gaming machines, cessation of gaming at an additional club premises or the surrender of a gaming machine licence, the Gaming Machine Act 1991 requires that gaming machines must be disposed of, either by sale or destruction within 1 month of the application being approved or the notification being accepted. Should you require further time to dispose of the gaming machines, an application to store gaming machines must be lodged with OLGR (see above).
Surrender
Submit the following to OLGR to surrender your gaming machine licence and place your operating authorities (i.e. if you operate as a hotel) in the tender for sale or your gaming machine entitlements (i.e. if you operate as a club) available for transfer:
