To apply for a new service contractor's licence (body corporate) submit the following to OLGR:

Form 14B: Application for a service contractor's licence (body corporate)

Form 20A: Affidavit for service contractors licence

company probity form
-
personal probity form (for each company director, company secretary and chief executive officer)

prescribed fee.
If you are a service contractor operating as a body corporate, you need to notify OLGR whenever a company secretary or chief executive officer ceases or commences employment:
