Individuals and organisations working as gaming machine 'suppliers' need a licence to operate in Queensland.

Application process

Apply

To apply for a licence, submit the following to the Office of Liquor and Gaming Regulation (OLGR):

  • Form 46: Application for a supplier's licence or the renewal of a supplier's licence (monitoring operator, major dealer, secondary dealer or testing facility operator).
  • Form 20C: Affidavit for suppliers licence
  • Company probity form
  • Personal probity form (for each company director, company secretary, chief executive officer, share holders and, where applicable, each beneficiary (over 18) under a trust).

Renew

To renew a licence, submit the following to OLGR:

  • Form 46: Application for a supplier's licence or the renewal of a supplier's licence (monitoring operator, major dealer, secondary dealer or testing facility operator)
  • Form 20C: Affidavit for supplier's licence
  • Company probity form
  • Personal probity form (for each company director, company secretary, chief executive officer, share holders and, where applicable, each beneficiary (over 18) under a trust).

Vary

You must notify OLGR if any of the following changes occur:

  • change of company name
  • change of shareholders
  • change of directors and/or secretary.
  • You must obtain approval for premises used for the purposes of operation and/or storage of gaming equipment.

Cancel

To surrender your licence submit the following to OLGR:

  • Form 49: Surrender notification of supplier's licence.

Transfer

This licence cannot be transferred.
