Gaming machine service contractor's licence (individual or body corporate) - Queensland

Description

You need this licence to install and service gaming machines in clubs and hotels on behalf of licensed monitoring operators (LMOs). All work carried out by the service contractor is carried out under contract for the holder of a supplier's licence (LMO).

Service contractors - as individuals or body corporate - need this licence to maintain gaming machines throughout Queensland.

Application process

Apply

To apply for a new service contractor's licence (individual) submit the following to the Office of Liquor and Gaming Regulation (OLGR):

  • Form 14A: Application for a service contractor's licence (individual)
  • Form 20A: Affidavit for service contractor's licence
  • personal probity form
  • prescribed fee.

To apply for a new service contractor's licence (body corporate) submit the following to OLGR:

  • Form 14B: Application for a service contractor's licence (body corporate)
  • Form 20A: Affidavit for service contractors licence
  • company probity form
  • personal probity form (for each company director, company secretary and chief executive officer)
  • prescribed fee.

If you are a service contractor operating as a body corporate, you need to notify OLGR whenever a company secretary or chief executive officer ceases or commences employment:

  • Form 19: Notification of cessation or commencement of secretary or executive officer of a service contractor (body corporate)

Transfer

This licence cannot be transferred.
