Health Services Permit - Victoria

Description

Your organisation may need this permit if your organisation intends to obtain and use scheduled medicines and controlled substances in the provision of health services. Relevant health services include medical and dental services, optometry services, hospitals, and aged care facilities. This permit relates to the use of scheduled medicines.

There are 4 different types of permits:

  • Type A (single site with no beds)
  • Type B (residential aged care with single storage facility or single site with 1 to 30 beds)
  • Type C (multiple sites with no beds or single site with 31 to 100 beds)
  • Type D (multiple sites or single site with more than 100 beds).

To be eligible for this permit you must:

  • be a fit and proper person
  • ensure your premises is suitable, sanitary and adequately equipped for providing health services.

Ongoing eligibility requirements

Ongoing requirements include:

  • paying the required yearly renewal fee
  • regularly reviewing relevant procedures, and completing an application to change details relating to the permit if they have changed.

Lodgement process

Please submit your application on the approved online form to the Agency accompanied by the required fee.

Approval time

A representative of the Department will need to inspect the premises of all applicants. The Department will normally process and make a determination about issuing a permit within two to three weeks after it has been confirmed that the applicant is acting in compliance with legislated standards.
