Health Services Permit - Victoria

Description

You may need this permit if your organisation intends to purchase or obtain and use any poisons or controlled substances for the provision of health services. Relevant health services include medical and dental services, optometry services, hospitals, and aged care facilities.

There are 4 types of permit:

  • type A (single site with no beds)
  • type B (residential aged care with single storage facility or single site with 1 to 30 beds)
  • type C (multiple sites with no beds or single site with 31 to 100 beds)
  • type D (multiple sites (with beds) or single site with more than 100 beds).

Please note, 'bed(s)' refers to facilities where beds are used for overnight patient accommodation.
