Health Services Permit - Victoria

Description

You will require this permit if you intend to obtain and use poisons and controlled substances in the provision of health services. Health services include medical and dental surgeries, optometrists, pharmacies, hospitals, and respite, nursing or aged care facilities. This permit covers administering a Schedule 8 or 9 poison or a drug of dependence.

Eligibility Requirements

To be eligible for this permit you must:

  • correctly complete and submit an application form
  • complete and submit an application to nominate a responsible person
  • complete and maintain a Poisons Control Plan
  • pay the required fee.

Ongoing Eligibility Requirements

Ongoing requirements include developing and maintaining a Poisons Control Plan (PCP).
