You will require this permit if you are a heath service and will be purchasing medication for the treatment of patients of your health service. A health service is a service that maintains, improves, restores or manages the physical and mental health and well-being of people.
You must nominate a person to have overall responsibility for each premises that is included on the permit. If an individual will hold the permit on behalf of the health service, they can also be the person nominated as having overall responsibility at any or all of the premises. Individual applicants and persons nominated to have overall responsibility for a premises must be registered through the Australian Health Practitioners Regulation Agency and have authority to possess and administer scheduled medicines under the Medicines and Poisons legislation. Suitable applicants include:
- medical practitioners
- registered nurses
- endorsed podiatrists
- endorsed optometrists.
Individual applicants and persons nominated to have overall responsibility for a premises must complete a Personal Information Form: Identification, Fitness and Probity. If the permit is to be issued to a partnership, or body corporate, each partner or corporate officer must complete the personal information form.
