Human Artificial Fertilisation Practice Licence - Western Australia

Description

You will require this licence if you intend to carry out an artificial fertilisation procedure. An artificial fertilisation procedure means any artificial insemination procedure, or in vitro fertilisation procedure. You will also require this licence if you intend to use a human embryo for a purpose other than reproductive technology treatment of the woman from whom the embryo was taken (including a research purpose).

This licence may be issued to a group of people who practise together, including a body corporate. You will require a separate Human Artificial Fertilisation Storage Licence to store human eggs, sperm or embryos.

If you are a medical practitioner, you may apply for an exemption which authorises you to carry out an artificial insemination procedure without a licence.

A licence defines the need to obtain recognition / certification and registration to undertake a certain business activity.

Eligibility requirements

To be eligible for this licence you must:

  • be a fit and proper person
  • be accredited to carry out reproductive technology by an approved organisation
  • establish that you require the licence to fulfil a genuine social need
  • have adequate and appropriate premises, equipment, staff supervision, support services, and facilities available
  • have adequate arrangements relating to the counselling of participants and obtaining an informed and effective consent
  • have adequate procedures in place for the compilation, storage and confidentiality of records.

If you are applying on behalf of a group of persons, or a body corporate, you must ensure that every person with a material interest in the practice, and every person in a position of authority is a fit and proper person.

You will not be eligible for this licence if you are an individual or organisation that is bankrupt, under receivership, in liquidation, or disqualified from holding a licence.
