Description

You will need this licence if you intend to store any of the following for an artificial fertilisation procedure:

  • any human egg intended for use in an in vitro fertilisation
  • any human egg undergoing fertilisation
  • any human embryo
  • human sperm, obtained from different men.

A licence defines the need to obtain recognition / certification and registration to undertake a certain business activity.

Eligibility requirements

To be eligible for this licence you must:

  • be accredited to carry out reproductive technology by an approved organisation.

If you are applying on behalf of a group of persons, or a body corporate, you must ensure that every person with a material interest in the practice, and every person in a position of authority is a fit and proper person.
