To obtain your registration you must:
- complete and submit your application form electronically
- provide any required information or documentation, including the names and contact details of all persons with an interest in the proposed pharmacy business, and copies of any relevant sale agreement, partnership agreement, lease, or security interest
- provide plans and specifications of the premises sufficient to show that the premises meet relevant requirements for the competent and safe practice of pharmacy, including demonstrating that the premises are not in, or accessible via, a supermarket
- provide any other information outlined by the Board
- pay the prescribed fee.
