Research or Education Permit - Western Australia

Description

You will require this permit if you will purchase medicines or poisons for the purpose of conducting research or education. This permit is usually granted to schools, universities, government departments or research organisations.

You must nominate a responsible person to have overall responsibility for each premises on the permit, who have a relevant qualification or experience handling the medicines and/or poisons on the permit. Applicants and persons nominated to have overall responsibility for a premises must complete a Personal Information Form: Identification, Fitness and Probity. If the permit is to be issued to a partnership, or body corporate, each partner or corporate officer must complete the personal information form.

1 year. A renewal application will be mailed to the postal address approximately 2 months prior to expiry.
