Declaration of Private Hospital - Australian Government

Description

You will require this declaration if you intend to operate a hospital where patients may claim expenses from the Pharmaceutical Benefits Scheme, Medicare Benefits Scheme or private health insurers.

You are required to apply to the Minister to make a declaration in regards to your hospital in the following circumstances:

  • commencement of a new facility
  • change of existing hospital's registered business name
  • change of existing hospital's address
  • amalgamation of existing hospitals.

Eligibility requirements

To gain a declaration you must provide a:

  • private hospital information form
  • copy of your state/territory hospital licence
  • copy of Hospital Casemix Protocol (HCP) Data to insurers
  • copy of Private Hospital Data Bureau (PHDB) data to the Commonwealth Department of Health
  • copy of an accreditation certificate or evidence that the accreditation process has commenced.
